A completely editable student info card, great for getting basic information from your teens or first time visitors (and for all you youth pastors who, like me, don't have fancy online registration systems). Bonus info about favorite snacks and hobbies give extra insight to small group leaders.

I print these on cardstock and have my teens fill out a new card each year, and small group leaders ask new visitors to fill one out at the end of the meeting. Go with the ready-to-go version or drop your youth logo in and it's ready to go, set up to print 2 per sheet (cards measure 5.5x8.5 inches) with evenly spaced margins, so one slice down the middle is all it takes for a perfect card.
